The percentage you pay of the allowed amount for covered health care services to providers who do not contract with your health insurance or plan. Typically 40 - 50%.

HMO and EPO plans do not include out-of-network coverage. Meaning you may need to pay all.

PPO and POS plans include out-of-network coverage.

Out-of-network coinsurance usually costs you more than in-network coinsurance.
